Overview

A requisition is a printable form that accompanies samples sent to a reference laboratory. It includes subject information, test orders, sample details, and billing information. Requisitions are generated per accession and can be customized by selecting which samples to include.
Requisitions are only available when your lab has the requisitions feature enabled. Contact your lab administrator if you do not see the requisition option.

Generating a requisition

Open the accession

Navigate to the accession detail view for the accession you want to generate a requisition for.

Open the actions menu

Click the actions menu (three-dot icon) in the accession header.

Select the reference lab

Hover over Print requisition and select the reference lab from the submenu. Only reference labs associated with the accession’s test specifications appear in the list.

Select samples

On the requisition preview page, use the sample selection sidebar to choose which samples to include. All samples are selected by default.

What the requisition includes

The requisition form contains the following sections:

Selecting samples

When an accession has multiple samples, you can choose which samples to include on the requisition. The sidebar displays all samples with their tube type and collection date. At least one sample must be selected.
